CDC is an approach for the management of care and planning for elderly people. It allows consumer and upkeeps to make a positive influence on the design and delivery of the service they receive with a greater degree of choice on the delivered service. Which means providers must;

  • Have an in-depth conversation with consumers about their needs & goal.
  • Develop co-care plans.
  • Offer greater transparency to consumers regarding the package service and funding.
  • Figure out the level of involvement of consumer in managing their packages.
  • Carry out formal re-assessment and monitoring to generate certainty on the existing packages.

 

Consumer Directed Care (CDC) are not limited to just a basic list of services. Client’s can approach for home care package funds to purchase the whole range of services including;

  • Support services i.e. helping inside the home, attending doctor appointments, and involving in social activities.
  • Clinical care such as nursing, allied health, physiotherapy and mobility.
  • Care coordination
  • Personal care such as dressing, showering, eating, grooming and moving around.

 

As per the Aged Care Act 1997, the home care agreement should include the following set of information

  • The type of care service provided to the client.
  • The levels of care service including the capacity of the care provider.
  • Policy’s and practice for allocating fees to the client.
  • Circumstances of suspending or terminating the service (penalty or certain percentage the clients must pay during the suspension/termination period)
  • The process of handling client grievances and complaints.
  • Responsibilities of client.

 

How to take Consumer Direct Care Packages (CDC) in Sydney, Australia?

While taking the service or during the on-going service period, you must;

  • Jointly work with the clients in order to update and co-produce care plans.
  • Explain the monthly statements and funding available under the package.
  • Regularly connect with the clients for the review of the experiencing services.
