What is My Aged Care?

When we talk about “aged care,” we’re referring to a broad range of support services designed to help older Australians live well as they age. My Aged Care is an umbrella term that covers all types of assistance, from light help around the house to complex medical care and everything in between. My Aged Care is the Australian Government’s official system for accessing aged care services. It connects you with government-funded support and approved care providers in your area.

My Aged Care isn’t a care provider itself; instead, it’s the system that:

  • Assess what kind of help you need
  • Connects you with government-funded aged care services
  • Helps you find approved care service providers in your area
  • Provides information and support throughout your journey

The services it can connect you, includes everything from someone to help with housework and gardening, to full-time residential care, and everything in between.

What are My Aged Care Services?

My Aged Care connects you to a wide range of Australian government-funded aged care services designed to support older people at different stages of their care needs. These services generally fall under three main categories. It’s important to note that the Australian aged care system is currently undergoing significant reforms, with new programs set to replace some of the existing ones.

Home Care Packages (HCP)

Best for: People with more complex or long-term care needs who want to stay at home.

Home Care Packages offer a higher level of support than CHSP, and funding is allocated based on assessed need. These are coordinated care packages funded by the Australian government and tailored to your unique needs. There are four levels of home care package support, from basic to high-level care, covering services like personal care, nursing, physiotherapy, and home modifications. You choose the provider, and they help manage your plan.

Important Note: The Home Care Packages program will be replaced by the new Support at Home Program from 1 November 2025. If you are currently on a Home Care Package, you will transition to the new program automatically.

Residential Aged Care Facility (RACF)

Best for: Seniors who can no longer live safely or independently at home.

Residential Aged Care Facilities (RACFs), also known as nursing homes, offer 24/7 care and accommodation for people who need constant supervision or specialised medical care. These facilities provide meals, personal care, accommodation, and medical assistance, and are ideal for people with higher care needs. Costs vary, but government subsidies help keep them affordable.

Note: While the new Aged Care Act (commencing 1 November 2025) will bring some changes to regulation and resident rights, the fundamental concept of residential aged care facilities remains the same.

Commonwealth Home Support Program(CHSP)

Best for: Seniors who need a little help to stay independent at home.

The Commonwealth Home Support Programme (CHSP) is an entry-level program that offers short-term or occasional support. It’s ideal for older Australians who can manage most tasks on their own but need assistance with certain day-to-day activities like cleaning, shopping, meals, transport, and personal care. It’s a flexible program where clients pay a small contribution based on their financial situation.

Note: The CHSP will transition into the Support at Home program no earlier than 1 July 2027. This approach is to allow providers more time to adjust. 

How Does My Aged Care Work? Step-by-Step

Let’s break down the process into simple steps:

Step 1: Check Your Eligibility

To access government-funded aged care, you first need to check if you qualify. In general, you may be eligible if:

  • You’re 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people)
  • You’re a younger person with a disability or other care needs not met through other programs

You can check eligibility by visiting my aged care website, myagedcare.gov.au or calling 1800 200 422. The team will ask about your health, daily routines, and any challenges you’re facing to determine if an assessment is needed.

Step 2: My Aged Care Registration

Once you’ve determined that you’re likely eligible, the next step is to register with My Aged Care. During the registration process, you will be asked for basic aged care information such as your name, Medicare number, medical history, and the type of assistance you believe you require.  

If someone is assisting you, such as a family member or carer, they can be designated as a representative. Following registration, My Aged Care will begin the process of assessing your needs and determining what types of care you are eligible for.

Step 3: My Aged Care Assessment

After you register, you can apply for an assessment.  This is an important step because it helps the government determine the level and type of assistance you require.

Your care needs are assessed either by:

  • Regional Assessment Service (RAS) – for CHSPAged Care Assessment Team (ACAT) – for HCPs or residential care

A trained assessor will visit you at home to talk through your needs and goals and determine what level of care is appropriate.

Note: From 1 November 2025, these two assessment processes will be re

placed by a new single assessment system as part of the new Support at Home program.

Step 4: Receive Your Approval and Referral Code

Once your assessment is completed, you will receive an approval letter outlining the types of aged care services for which you are eligible. This letter will also include a unique aged care referral code for your approved services. This code serves as a key, allowing service providers to access your care plan and begin assisting. 

If you are approved for a Home Care Package, you may be added to a national waitlist based on your priority level.  Keep your letter and referral code safe; you’ll need them when selecting and contacting your preferred healthcare providers.

Step 5: Choose Your Services

Now that you’re approved, it’s time to choose who will provide your care. My Aged Care has an online “Find a Provider” tool where you can search for services in your area, compare providers, view costs, and read reviews. Whether you need help with household tasks, personal care, transport, or nursing support, you can select the provider that best suits your needs.

You have the freedom to choose one provider or a mix of providers, depending on the services you’re after. Once you find a suitable match, you can contact them directly, discuss your needs, and give them your Aged care referral code to begin the service setup.

Step 6: Develop a Care Plan

After deciding on a provider, you’ll collaborate to develop a personalised care plan. This is a detailed document that outlines the types of support you’ll receive, how frequently it will occur, and your goals, such as improving mobility, maintaining independence, or receiving medication assistance.  

Your care plan is customised to suit your lifestyle, preferences, and budget.  You will also sign a Home Care Agreement that outlines what the provider will provide, your rights and responsibilities, fees, and how to make changes if necessary.  A good care plan ensures that everyone is on the same page and that you are receiving appropriate support.

Step 7: Receive Care and Support

Once everything is agreed upon, your care will begin. Depending on your level of need, you may be eligible for services through the Commonwealth Home Support Programme or a Home Care Package. Your provider will coordinate visits, send staff to your home, and ensure that services are delivered according to your care plan. 

Cooking, cleaning, personal care, therapy, equipment, and even social support may all be included. If you are in residential care, the aged care facility will manage your daily care, housing, and health needs.  In either case, the goal of support is to assist you in living safely and comfortably. 

Step 8: Keep in Touch with My Aged Care

Your circumstances may change over time, and your care should adjust accordingly. That is why it is necessary to keep in touch with your care provider and My Aged Care. If your needs change, you may request a reassessment for a higher level of assistance.  If you are dissatisfied with your current provider, you can switch to a different one.

 You can also change your care plan if your health improves or you want to switch services. Reviews are typically conducted every 12 months, but you can request one sooner if necessary. Staying proactive puts you in control of your care and ensures you always receive the appropriate level of support.
